The goal of the Diabetes/Wellness Program is to provide health education and fitness programs to improve the health and
well-being of the community we serve. The program assists patients
in developing lifestyle skills for disease prevention and to limit complications of diseases such as Diabetes, Hypertension, Hyperlipidemia and Obesity.

A Certified Personal Trainer oversees all components of the Wellness Center including instruction of group exercise classes and individualized exercise plans. A Healthy Living Program Assistant provides clerical and instructional support for both the Diabetes Program and Wellness Center.
The Wellness Center is free to all clinic patients as well as any person with a CDIB card, persons working for a Tribal affiliated facility and Veterans. Participants are expected to provide a copy of eligibility and complete a basic application related to health, emergency contact and release of liability.
