Patient Eligibilty Guidelines

Northeastern Tribal Health System

accepts all American Indians and Alaska Natives of all tribes. To be eligible for services, you must have a CDIB card.

All of the following are required:

• CDIB or Tribal Membership Card
• Social Security Card
• Private Insurance Card (if applicable)
• Valid Photo ID
• Patients under 18 must have their original State Certified Birth Certificate (no copies).

All minors must be accompanied by their parent or legal guardian on the initial visit.
